Children's Continuing Care Services

The Children’s Continuing Care Manager has responsibility for the process of continuing care, working closely with the team managers, healthcare professionals and partner agencies to provide expert advice and training

The CCNS  are responsible for providing bespoke packages of care to children following an agreement from funding panel.

In conjunction with Serennu Children’s Centre the CCNS facilitate community play clubs where continuing care children can socialise with their peers and enjoy fun activities.

To Make a referral for Continuing Care

Healthcare professional must complete a children and young people’s continuing care referral form and email it to the Continuing Care Manager:
